Which specialist would be best for shingles? Neurologist or dermatologist? For an immune compromised pt?

Pt is 57 y/o male with a non-aggressive blood cancer. Has been active, working, receiving chemo. Developed a severe case of herpes zoster. Treated so far by his oncologist, but I don’t think that doc has beenaggressivee enough or anticipated the complications that occurred. (Not sure if he even put pt on vancovir or prednisone.)

So which specialist would be best equipped to Tx this pt.

He developed complete urinary retention (from opiates?). May have more than 3 dermatones involved.

Also – are there any leading edge treatments for herpes zoster? I am familiar with the various website info.

You know, I was told by my neurologist less than 6 months ago that there *is* something new some docs are using for shingles, but she did not mention what it was. It came up because she finally noticed in my chart that I had an outbreak when I was 25–she wanted to verify that it was, in fact, shingles, since that’s an unusually young age.

A dermatologist, or even a GP, for that matter, can treat the rash and the pain as well as any oncologist. I would suggest a neurologist, as he or she might have some other ideas for treating the neuralgia and pain, rather than just throwing opiates at the patient. I actually know one person, elderly, who was sent to an anesthesiologist last year and given an epidural for the residual pain from a shingles episode. They were referred by a neurologist.

I think that would be the best bet. It is, after all, primarily a neurological problem, not dermatological. I will try to contact the doc who spoke about the new thing some are trying for shingles, and I will come back and add an edit if I am able to get some information from her for you.

Do we have an Immunodeficiency syndrome?

My mother had varicella a long time ago. Then she had pneumonia and encephalitis one year after that. The encephalitis was really bad, she couldn’t speak and write for one year and she had necrosis on her brain. Our medical system is quite bad. Luckily my aunt had money to buy treatment and she only has a slight speech problem – talks slower, has some lapsus. But she is fit to work and her judgment and movement are normal. After 7 years she had necrosis on her femur head, she did surgery… But that’s another story, it happened from the Hydrocortisone she took when ill.
I had chickenpox when I was in primary school, then, about 4-5 years later (can’t recall) I had Herpes Zoster. Luckily I didn’t experience postherpetic neuralgia afterwards until now, (I am 21).
Does this mean we definetly have a genetic immunodeficiency? Or isn’t it a rule? Should I get tested to find out?
If we do, what other complications may we experience?
P.S. I also had a really severe case of laryngitis back then, for 2 weeks or so, where I’d choke all the time, stay in bed with a wet drape at the window, but I’m not sure that’s relevant.

It is possible, and there are genetic immunodeficiency disorders, but that is something that should be tested for. However, it’s not an automatic diagnosis, and there are other factors which could cause a predisposition to infections such as the ones you mention. Herpes zoster (assume you mean shingles) at such a young age is unusual though, so I’d request follow up with the issue.

Your Questions About Shingles

Can you have Shingles without the pain??

I went to the rudest doctor ever on Tuesday regarding a red, bumpy, slightly itchy rash on my lower back/upper butt area. Long story short he told me to go home and look up my symptoms on the internet to see if they match Shingles and go from there. Umm… shouldn’t he be diagnosing me, not the internet? F#$%
Anyway so that’s what I did and my symptoms DO match shingles only I don’t have the extreme pain I keep reading and hearing about from anyone who knows anything about shingles.
He gave me Valtrex (omg) which is also used for Herpes Zoster AKA Shingles… but honestly don’t you think a doctor shouldn’t give you meds til he knows what the problem is???
Sorry to rant but my real question is: Can I still have shingles and not experience the pain and if so is it really bad that I can’t get to a doctor for 2 more days, making it nearly a week that I’ve seen the bumps? (They say get treatment w/in 3 days of seeing the bumps otherwise it’s harder to cure.)

I was diagnosed with Shingles, but I don’t have any pain – just ANNOYING ITCHING!! AHH!!

I did some online research, and it appears you can have shingles without pain – it all depends. Some people have mild itching and some people have “oh my god it hurts really bad” symptoms.

I would seek a 2nd opinion from another doctor ASAP for the reasons you mentioned above, it’s harder to treat shingles after 72 hours.

(I was prescribed Valtrex too, now everyone at Walgreens probably thinks I have genital herpes since I’m only 30 and shingles occurs mostly in older people. Great.)

Ladies…Do you find that you get inferior medical treatment compared to your spouse?

It seems to be consistent that when my wife has a medical problem, the doctors (this has happened with several) tend to downplay it and don’t give her medication (shingles is just one example) or referral to a specialist (numbness in the legs and shooting back pain.)
But when I see the doctor with the same symptoms I get sympathy, ample medication and referrals to specialists.
Many women that we know have voiced the same complaint.
What gives?

I presented to the doctor several times with complaints and was ignored. So I went to another and was ignored again. Then I went to another. Ignored again. When my condition got so bad that it was observable to any passerby on the street, someone finally paid attention and diagnosed my condition, which turned out to be chronic and serious. Another time I went to the doctor after surgery and told him I was feeling very sick. He ignored me. By the end of the day I was in so much pain I went to the emergency room and was admitted to the hospital for three days. When I was in the hospital, I noticed that I was treated much better and more respectfully when my husband was in the room. If he wasn’t there, I was told things like, “Why can’t you just be quiet and figure out that this is the way it is.” I ended up filing complaints, and the hospital gave me a five dollar gift certificate to the gift shop. I hate doctors. And you know what doesn’t make sense, is that I get treated poorly by young and old doctors, so whatever training they’re getting has not improved over the years apparently. Of course now they know that I actually have a medical condition, so they treat me better because I have good health insurance and I’m worth money to them. Bastards.

i don’t know if this will work but i have found L_LYSIN to be very helpful for all types of Herpes symptoms.?

i bought L-Lysin from a health store it is an amino acid it has stopped the breakouts of cold sores if i take it at the earliest signs of it like itching ,burning etc. (Herpes type one) and helps the pain of cold sores. You may consult your doctor but it seems to work with me and few other people have told me the same. it may be worth trying for other types of post herpetic symptoms but it is not a treatment for the outbreak of shingles or any other types of herpes it seems to stop the outbreak if taken early enough but does not cure or treat the disease.

Actually, people have been using L-Lysine for cold sores, genital herpes since the ’80s with mixed results. Pretty well documented on medical/home remedy sites since then.

I was told 32 years ago I had herpes simplex, and my treatment was simple?

My Mom had shingles, and I started getting symptoms at 22 years old. I know it comes from Chicken Pox. When I had the “belt” come on, I soaked balls of cotton in Rubbing Alcohol(Isopropyl) and taped them on to the sores. The waist sores stopped, but I then started geting them just above my knee. So I gave them the same treatment. I haven’t had an attack now anywhere in 12 years. All the drugs being hawked can’t do what Rubbing Alcohol can do!!!!! I have two brothers that are Doctors, and they can’t explain. Can anyone ??!!!
